Merrill Explosion Investigated by OSHA

The Occupational Safety and Heath Organization has now started investigating the events leading up to the explosion that took place at the Merrill manufacturing plant. OSHA has cited Okeelanta Corp. in South Bay, Fla

OSHA (U.S. Department of Labor's Occupational Safety and Health Administration) has proposed penalties $45,775 and cited Okeelanta Corp. in South Bay, Fla for repeated and serious violations of OSHA rules. The cited company has exposed workers to lacerations and amputation injuries. OSHA's Site-Specific Targeting Program incited the inspection done that payed more attention in social control efforts on comprehensive safety inspections where the highest rates of injuries and illnesses occur. The inspection found that Okeelanta has repeated violation that done before also. For this repeated violation, OSHA has proposed penalty of $12,500. The inspection has disclosed the violation includes failing to enclose sprocket wheels and chains on the specific machinery and also exposing employees to rotating parts and pinch points. Chicago-based ERA Valdivia Contractors Inc. to be penalized for health violations

The U.S. Department of Labor's OSHA has cited an industrial painting and sandblasting company, ERA Valdivia Contractors Inc. A total $130,300 has been proposed for exposing the workers to a dangerous lead material. After inspecting the company in 2009, OSHA has come to a conclusion that the company was wrongful for their act and they have been cited with two willful violations and OSHA has proposed a penalty as they have failed to provide an adequate protection equipment to the employees. The employer of the company has also been cited with 8 serious citations and $18,300 penalty has been proposed. The citations given addressed the company’s alleged failure to protect the workers and failing to ensure their safety on the work site. OSHA proposes fine of $61,200 against Derry, N.H., Contractor at Medford, Mass

Andover, Mass OSHA cites Tri-Sprinkler Corp. for the alleged willful and serious violations of the safety standards following the fall of a worker from ten feet at a Medford, Mass., worksite. The Derry, N.H based contractor faces $61, 200 in the proposed fines. The inspection by OSHA began on Sept. 29, 2009 after knowing the accident in August in which employee fell and was seriously injured when installing a sprinkler system above the ceiling of a building that was located at 215 Harvard St. In the inspection OSHA found that no form of fall protection had been provided that is needed for the employees who work six feet or higher. "The need for fall protection was clear and recognized, yet this employer did not provide this basic, commonsense and legally required safeguard," said the area director of OSHA for Middlesex and Essex, Paul Mangiafico. Marcus Hook, Pa The Occupational Safety and Health Administration( OHSA) has fined $32,000 to Sunoco Inc. in Marcus Hook for workplace safety and health violations. OSHA started the investigation following an ethylene complex explosion in May 2009 at the Marcus Hook refinery. As a result of investigation, the company was issued one repeat violation with a penalty of $25,000 and two other serious violations with a penalty of $7,000. "It is imperative that Sunoco correct all the identified hazards to ensure a safe and healthy workplace for its workers," said OSHA’s area director in Philadelphia, Al D’Imperio. The repeat violation includes the failure of the company to establish and implement the written procedures to maintain the on-going mechanical integrity of process equipment.
